Using the Contact Update screen you can change or add new details pertaining to contacts for each child.  Once you have made the changes you require, or entered the new details, click on the "Update" button and you will be returned to the Contact View screen.  If you do not wish to update the record, you can click the "Cancel" button.

 

Relationship

Please enter the relationship that this contact has to the child e.g. Uncle, Father, Mother etc.


Title (Optional)

Please enter the contact's title e.g. Mr. or Mrs. or Dr.


First Name

Please enter the contact's first name.


Last Name

Please enter the contact's last name.


Middle Name

Please enter the contact's middle name. If the contact has no middle name or it is not known, this field may be left blank.


Same address as

If the address details for this contact are the same as one of the other contacts associated with this child, you will be able to select from the drop-down box the address which is the same for this contact. The address details below will then be automatically populated for you.


Address

If you have not selected an option from "Same address as" above, begin typing the address into this field. As you type, the system will suggest addresses from a list of valid New Zealand addresses. Click on the correct address suggested and the remainder of the address fields will be automatically populated.


Input address manually

Check this box if the address is not available in the “auto-complete” Address field.

 

Line Address 1

Only required to be entered if you have opted to input the address manually.

 

Line Address 2

Only required to be entered if you have opted to input the address manually.


City

Only required to be entered if you have opted to input the address manually.


Postal Code

Only required to be entered if you have opted to input the address manually.


Country

Only required to be entered if you have opted to input the address manually.


Daytime Telephone

Please enter the daytime telephone number for this contact.


Evening Telephone

Please enter the evening telephone number for this contact.


Mobile

Please enter the mobile telephone number for this contact. If you have subscribed to the GoText! text message system you will need to enter mobile numbers using the international standard format including the country code and the area code without the leading 0. The country code for New Zealand is 64. For example, a parent might say their mobile number is 021 123 456. This should be entered as +6421123456.


Fax

Please enter the fax number for this contact.


ID

Please enter the ID number associated with this contact e.g. Passport Number, Drivers License Number etc.


ID Type

Please enter the document type for the ID number entered above e.g. Passport or Drivers License


Email

Please enter the email address for this contact.


Occupation

Please enter the occupation for this contact.


Comments

Please enter any comments you may wish to record about this contact.


Email Financial

If you would like this contact to receive invoices, statements, credit notes and etc. by email, please check this box.


Email Letters & Parent Portal

If you would like this contact to receive letters and updates about the parent portal content by email, please check this box. If you subscribe to the Infocare Parent Portal option, a password for access to the Parent Portal will automatically be created for this contact, and you will be prompted to see if you would like to send a Parent Portal welcome letter to this contact.


Text Message

If your centre has subscribed to the GoText! text messaging system and you would like this contact to be able to receive text messages, please check this box. You will need to enter mobile numbers using the international standard format including the country code and the area code without the leading 0. The country code for New Zealand is 64. For example, a parent might say their mobile number is 021 123 456. This should be entered as +6421123456.


Bank

In order to make receipting more efficient, the Infocare system can record banking details for the contact person responsible for payment of the fees. This will enable the billing module to provide default bank and branch information for each contact when receipts are generated. This information can be over-ridden at receipt creation Time if required.

Please enter the branch details for the bank specified above.


Primary Caregiver

Check this box if this contact is the child's primary caregiver.


Use as Emergency Contact

Check this box if this contact is to be included on the list of people to contact in the case of an emergency for this child.


Forbidden/Conditional Access

Check this box if this contact has conditional access or is forbidden access to this child.


Allowed to Collect

Check this box if this contact is authorised to collect the child.


Update

Please click the "Update" button if you are satisfied with the information you have entered about this contact and you wish to update the record accordingly.


Cancel

Please click the "Cancel" button if you wish to discard any information you have entered regarding this contact and return to the previous screen.
