This screen enables you to view and update an employees Covid-19 vaccination information.


Select the appropriate status from "No information provided", "Fully vaccinated", "Partially vaccinated", "Status unknown" or "Exempt".

If you select "Fully vaccinated", both Dose 1 and Dose 2 must be recorded; or is you select "Partially Vaccinated", Dose 1 must be populated.

Exempt Date

If the employee has an exemption, record the date on the exemption document.


Use this field to record any comments such as the date information was requested from an employee etc.


Two fields are provided to record information of Dose 1 and Dose 2.


If the employee has received either Dose 1 or both doses, select either "Administered" or "Administered (date unknown)".


If you have been provided with the date either Dose 1 or Dose 2 was administered, enter the date in this field.


Click "Update" to save the changes.
